My Medical Assistant ™ is an all-in-one organizer designed to allow you to personally maintain your medical information and health care history. It contains easy-to-use forms for medications, hospitalizations, illnesses, cholesterol, weight, and dental and optical care, to name a few. Many of the forms were based on new patient questionnaires and other documents used by doctors' offices. The standard edition is presented in an easy-to-use 8-1/2" x 5-1/2" three-ring binder so pages can be added, as you need them. It is small enough to fit into an oversized purse, briefcase, diaper bag or suitcase.

Never again will you worry that you won't have critical information at your fingertips when loves ones are dealing with a medical emergency, coping with multiple health problems, traveling away from home, or handling any number of out-of-the-ordinary medical situations.

GALLOP FOR A CURE – Snowhill, MD

“I was diagnosed with MS in 1995 and and wanted to be proactive about raising money for MS research hoping one day a cure would be found. We have just completed the 2nd annual Gallop for a Cure and to date, have already raised over $13K.”- Christi Medice

GOLF TOURNAMENT

“Since 1997, Mayfair Jewelers has been proud to hold an annual golf tournament to benefit Multiple Sclerosis. The Mayfair Jewelers ‘Family’ has been personally affected by this disease and we are eager to help find a cure. We chose The Montel Williams Foundation because all the proceeds go toward research. Our tournament brings together golfers from all over the capital region who support the efforts to cure MS. The day is full of great friends, food, golf, gifts and lots of fun.”

To date, Mayfair Jewelers have raised almost $38K.

LAWN AND GARDEN TRACTOR COMPETITION

Dana has been bringing together a local group of “good old boys” for a day of competition in their lawn and garden tractor-pulling club. They have a great time while raising money for local charities and organizations.

Dana’s team, Dana’s MS Defeaters have raised almost $12K for MS research.
